PRIVACY ACT 2020 AND AML/CFT ACT AUTHORISATION

I/we authorise Smartcover Finance Limited and other associated third parties ("Smartcover") to collect, retain and use personal information about me/usfor the purposes of:

• Assessing my/our credit worthiness;
• Attending to the financing, whether directly or indirectly, of my/our contract(s) and enforcing Smartcover's rights thereunder;
• Administering and managing my account and any credit provided by Smartcover;
• Confirming information provided by me/us is true (including updates on information provided);
• Undertaking customer due diligence and monitoring in accordance with the Anti-Money Laundering and Countering Financing of Terrorism Act 2009 (AML/CFT Act);
• Compliance with any legislative, regulatory or other legal obligations; or
• Marketing and/or informing me/us about the goods and services provided by Smartcover.

I/we authorise Smartcover to make enquiries about me/us and collect personal, financial and commercial information about me/us for any of the above purposes from:

• Credit reporting agencies, Credit providers, my/our employer(s), accountant(s), government agencies including Immigration NZ, relatives, or any other person(s) Smartcover considers appropriate for the said purposes.
• We will provide your personal information to Centrix Group Limited when we use their consumer credit reporting services (for example for credit checking).

• Centrix will collect your credit information for the purpose of providing credit reporting services to us and other subscribers to Centrix consumer credit reporting services, and Centrix will use and disclosure your credit information in accordance with the Centrix Privacy Statement. You have rights of access to and correction of your credit information and any other personal information held by Centrix as set out in Centrix Privacy Statement

I/we authorise Smartcover to provide information:

• To any person for the foregoing purposes;
• To any employees and agents of Smartcover and/or any other person, in any ordinary course of business, for any of the foregoing purposes;
• To credit agencies for the purpose of maintaining proper or effective records.

I/we authorize other persons to provide information to Smartcover in response to their enquiries made for any of the above purposes.

I/we further acknowledge that pursuant to the Privacy Act 2020, I/we have a right of access to information collected by Smartcover about me/us and to request that this information be corrected.

The information collected and retained by Smartcover will be held at the office of Smartcover Finance Limited, 3/2, Unit E, Ceres Court, Rosedale, Auckland or the registered offices of any party nominated by Smartcover Finance Limited.

I/we also authorise Smartcover to:

• Use customer due diligence services (including electronic based services) to verify my/our identity and undertake customer due diligence and monitoring in accordance with the AML/CFT Act; and
• Obtain credit information from credit reporting agencies to make credit decisions affecting me/us (and for directly related purposes including debt collection).

I/we acknowledge and agree that:

• When using such services, Smartcover and the service provider will each disclose and receive information about me/us for that purpose, and the service provider may hold information on its system and use it to provide its customer due diligence or credit reporting service to its other customers.

If I/we default in my/our payment obligations, information about that default may be given to credit reporters (and used by them to provide credit reporting services to their customers).
